enroll for dental benefits through bi-weekly payroll
deduction only.
All applicants must complete a payroll deduction form
and an enrollment form for the plan they wish to sign-up
for. Submit completed forms via

1. The following individuals must
enroll for dental benefits through monthly direct-billed
credit card payments only:
- LIUNA Members on Military Furlough
- LIUNA Retired Members
- CA ARNG Supervisors and ANG Technicians

2. Once payment information is
completed you
will be taken to the plan enrollment form. Please
complete the form and submit it using one of the
following methods.

3. Prices include dental premiums and
applicable membership dues:
- Members on Military Furlough are dues-exempt.
- Retirees dues are set at 50% of the regular
rate.
- All other must pay full membership dues.
