Request for Personal Items

If you are a Title 5 or Title 32 employee of the Department of the Army or the Air Force, including the National Guard Bureau or the Army and Air National Guard, and are in need of assistance requesting personal items that you are required to wear as a condition of employment, such as military uniforms, footwear, cold weather gear, etc., please fill out this form. A representative will contact you as soon as possible to discuss your concerns and your options.

SECTION 1 - PERSONAL INFORMATION

The information provided is strictly for the purposes of representing you and will only be shared with individuals who have a need-to-know.




SECTION 2 - CIVILIAN POSITION INFORMATION

This information can be found in your latest SF50. You can view a copy of your SF50's on MyBiz. 



Supervisor Information

Please provide contact information for your immediate supervisor.



 SECTION 3 - DUAL STATUS TECHNICIANS (DSTs)  

 If you are not  a Dual-Status Technician please skip to SECTION 4.  


Military Position Information

If you are a Dual-Status Technician please provide information for your military position. While not required, providing this information can help us better understand your complaint, and it can also help us determine what the best course of action may be to achieve successful resolution. 




SECTION 4 - ITEMS BEING REQUESTED

In accordance with (IAW) Article 10 of your respective state/territory's collective bargaining agreement (CBA), each DST is entitled to received four (4) sets of their civilian duty uniform. This includes all accessories required for proper uniform wear IAW your respective military branch's regulation, to include undershirts, socks, etc.

 Membership Requirement  

Assistance with requesting required personal items is a service available to Union members only. If you are not a member but intend to join the Union, you may proceed to submit a request. You will be required to join the Union prior to your request being processed.
